To help us get started with your care as efficiently as possible, please bring the following items to your first visit:
Referral or Prescription
(If required) from your physician, ARNP, or PA for physical therapy.Photo ID
Such as a driver’s license or other government-issued identification.Insurance Card
We will make a copy for our records.Accident or Workers’ Compensation Info(if applicable)
Please bring contact details for your case manager or insurance adjuster, along with your claim number.
What to Wear
Please wear comfortable clothing that allows easy access to the area being treated.

We are in-network with the following insurance plans:
BCBS PPO
Blue Shield Federal
Medicare Part B and its secondaries (including BCBS, BCN, Aetna, United, etc.)
Most Medicare Advantage plans (Priority, BCBS, Aetna)
UnitedHealthcare Medicare Primary
We also accept some HSA plans and certain Aetna plans, depending on coverage.
Insurances We Do Not Accept
UnitedHealthcare for patients under 65
HMOs of any kind
You are encouraged to contact your insurance company directly before your appointment to verify your coverage.
Additionally, during your initial consultation, we can contact your insurance provider on your behalf to help clarify any potential out-of-pocket costs.

No Insurance? No Problem.
For patients without insurance coverage, we offer the option to pay out-of-pocket for physical therapy services.
Payment is due at the time of service. Please contact us for current rates or to discuss your treatment plan.

Cancellation Policy
We do our best to accommodate every patient’s schedule. To help us serve all of our patients effectively, we kindly ask that you provide at least 24 hours notice if you need to cancel or reschedule your appointment.
This allows us the opportunity to offer your time slot to someone on our waitlist.
Late cancellations or no-shows may be subject to a cancellation fee, as the time was reserved specifically for you.

Payment Policy
We accept cash, debit cards, and credit cards for payment.
It is our policy that payment is due at the time of service.
If you are using insurance, we will verify your benefits prior to your second visit. At that time, we will inform you of any copay, coinsurance, or out-of-pocket responsibility.

Your Personalized Plan of Care
Every patient is unique, and so is every diagnosis. That’s why, in collaboration with your referring physician (if applicable), your physical therapist will develop a customized plan of care tailored to your specific needs and goals.
During your initial evaluation, your therapist will:
Assess your condition
Determine the appropriate frequency and duration of treatment
Explain your recommended plan of care and expected outcomes

What to Wear to Your Appointment
Please dress comfortably in clothing that allows your therapist easy access to examine and treat the affected area.
For treatment sessions, we recommend:
Loose-fitting, comfortable clothing suitable for gentle exercises
Shorts for knee, hip, or lower back issues
Tank tops or sleeveless shirts for shoulder or upper body issues
Supportive sneakers for stability during movement or exercise
Wearing the right clothing helps us give you the best care possible.
